Transitional Care
Coming home from the hospital can feel scary. We help make sure your recovery goes smoothly and nothing gets missed.
What We Do
- Go over your hospital papers and instructions
- Check your medicines and make sure they are right
- Set up any follow-up tests or visits you need
- Talk to your other doctors to keep everyone on the same page
- Watch how you are healing
- Answer your questions about what to do next
When to Come In
Try to see us within 1 to 2 weeks after leaving the hospital. The sooner the better — many people end up back in the hospital because they waited too long for follow-up care.
